About Us
Founded in 1951, the Ohio Physical Therapy Association (OPTA) is non-profit professional organization representing licensed physical therapists, licensed physical therapist assistants and students enrolled in physical therapy and physical therapist assistant programs in Ohio. OPTA represents more than 3,800 therapists and therapy students in Ohio. This organization is a chapter of the American Physical Therapy Association, a national professional organization representing 100,000+ members.

MISSION
Our mission is to provide the driving force for advancing physical therapist practice.

VISION
Our vision is for Ohio to be the model of excellence, led by the advocacy, leadership, and expertise of the members of the OPTA.

In order to achieve our Mission and Vision, we have established the following goals:

Our Goals
Recruit, mentor, and retain members and leaders
Advocate for health and governmental policies that advance PT practice
Improve the reimbursement climate for PT services
Increase public awareness of the practice of PT
Provide premiere professional development and programming
Advance the science of PT practice

These goals were developed as part of the OPTA Strategic Plan and last updated May 2017.

Each year OPTA honors outstanding achievements in the OPTA physical therapy community by recognizing members who have excelled, achieved and surpassed expectations in a number of areas. Award recipients are recognized annually with a ceremony at OPTA's Annual Conference. Awards are presented for the previous calendar year.The deadline for nominations is Friday, Mar. 6!

AWARD DESCRIPTION
To acknowledge a Physical Therapist who has made significant contributions to the profession of physical
therapy in the state of Ohio in one or more of the following areas: clinical service, education, research,
administration, public service or contributions to the organization.

NATURE OF AWARD
A plaque will be presented at the OPTA Annual Recognition Meeting & Luncheon on April 25, 2020. An
official announcement of the award will appear in OPTA's newsletter, ACCESS, and will be posted on the
OPTA website.

CRITERIA FOR SELECTION
1. Nominee must be a member in good standing of the OPTA.
2. Contributions may be in one or more of the following areas: clinical service, education, research,
administration, public service or contributions to OPTA.
3. Contributions must be of outstanding nature.

PROCEDURE FOR NOMINATION
• Complete online submission form to submit your nomination; all nominations must be
submitted online, along with one support letter attached to the message.
• Nominations must be submitted by Friday, March 6, 2020.
The nominator is responsible for submitting the following documents
• One letter of support focusing on the strengths of the nominee in relation to award criteria
Notification of receipt of nomination packet
• After successful submission of the nomination, the nominator will receive a confirmation email.
• Once the nomination is received, the nominee will be contacted to obtain their Curriculum
Vitae/Resume
